Appointment Requirements

Agents wanting to join First Benefits Insurance Mutual, Inc. please contact Peg Pirrotta or click here for an agent packet.

The appointment is simple. All you need is:

• A copy of the NC P&L License for each agent placing business with FBI
• Appointment of NC Agent form for each agent placing business with FBI
• A copy of the declaration page of your E&O policy
  ($1 million minimum required)
• A completed W-9 Form (see www.irs.gov/pub/irs-pdf/fw9.pdf for instructions)
• A completed Agency Agreement
• (If incorporated) A copy of your corporate license

Upon receipt of the completed packet, we will mail out to each agent appointed the underwriting guidelines.
